Incident Reporting

Your safety and the safety of our clients is important. This page is used to report any incidents that occur while you are providing care.

If an incident happens, please complete the Incident Report Form as soon as possible—preferably the same day.

What Should Be Reported?

Report any unexpected event, including:

  • Client or aide injuries

  • Falls or near-falls

  • Unsafe home conditions

  • Medication issues or missed reminders

  • Behavioral concerns or conflicts

  • Accidents, complaints, or anything that didn’t feel right

If you’re unsure, submit a report anyway.

What Happens Next?

  • Your report is sent directly to our company email

  • Our team reviews it promptly

  • We will contact you if follow-up or support is needed

  • All reports are handled professionally and confidentially

Reporting an incident does not mean you are in trouble.

When to Submit

  • Immediately for urgent situations

  • Same day, whenever possible

  • No later than 24 hours after the incident

For emergencies, call 911 first, then notify the office and submit this form.
